Physical & PPD Test (must be completed within 6 months of application)

Chest X-ray (if applicant has a positive PPD test history)

CPR (Card must indicate adult CPR. Adult and Child but not infant CPR as a stand alone)

First Aid

Home Health Aide Certification & Skills check list from accredited school in the District of Columbia

Medicaid provider number (for home health aides only)

For HHAs - current HHA license (license must not have any restriction)

For RNs & LPNs - current RN or LPN license (license must not have any restriction)

High school diploma or equivalent or higher (for DSPs only)
